About QAPIShield

Built by Nurses. Designed for Safety.

"To help skilled nursing facilities prevent adverse events, improve survey outcomes, and reduce CMS penalties through predictive, nurse-designed AI."

— Our Mission

Our Story

QAPIShield was born in the hallways of skilled nursing facilities, not in a Silicon Valley boardroom. Our founders spent years as frontline nurses, MDS coordinators, and DONs—watching preventable adverse events happen despite everyone's best efforts.

We saw residents fall because risk factors weren't connected. We watched pressure ulcers develop because early warning signs were buried in paper charts. We experienced the heartbreak of hospital readmissions that could have been prevented with earlier intervention.

And we lived through surveys where months of hard work couldn't be demonstrated because our QAPI data was scattered across spreadsheets and binders.

We built QAPIShield to solve the problems we faced every day—to give clinical teams the predictive intelligence they need to prevent harm before it happens.

Why We Built QAPIShield

Skilled nursing facilities face unprecedented challenges. We built QAPIShield to address the systemic issues that put residents at risk and facilities under pressure.

High CMS Penalties

Skilled nursing facilities face penalties up to $22,000 per day for serious deficiencies. A single adverse event can trigger immediate jeopardy citations and devastating financial consequences.

$22K+per day in potential penalties

Survey Pressure

State surveyors scrutinize every aspect of care delivery. Without real-time visibility into risk trends, facilities struggle to demonstrate proactive QAPI programs during unannounced surveys.

68%of SNFs cited for quality deficiencies

Staffing Shortages

The national nursing shortage means fewer experienced eyes on high-risk residents. Overworked staff need tools that surface critical risks before they become adverse events.

94%of SNFs report staffing challenges

Inconsistent Documentation

Manual care planning is time-consuming and error-prone. Inconsistent documentation leads to survey citations and missed opportunities for early intervention.

QAPIShield is the Solution

We built a platform that predicts risks before they become adverse events, generates survey-ready documentation automatically, and gives clinical leaders real-time visibility into their QAPI programs—all without requiring PHI or complex EHR integrations.

Our Core Values

These principles guide every decision we make—from product development to customer support.

Resident Safety First

Every feature we build starts with one question: will this help prevent harm to residents? Clinical outcomes drive our product decisions.

Nurse-Designed

Our platform was built by nurses who understand the realities of SNF care. We design for the workflow, not around it.

Evidence-Based AI

Our algorithms are grounded in peer-reviewed clinical research and validated against real-world outcomes data.

Privacy by Design

We never use PHI. Our risk models work with de-identified clinical factors, ensuring HIPAA compliance without compromising accuracy.

Actionable Insights

We don't just identify risks—we provide specific, evidence-based interventions that staff can implement immediately.

Continuous Improvement

Our models learn and improve over time, incorporating the latest clinical research and real-world outcome data.

Our team combines decades of clinical experience with deep expertise in healthcare technology.
